• The Transfer Workgroup - a multidisciplinary improvement team - was formed to develop a standardised care processes for the transition of patients between the CICU and the cardiac step-down unit and reduce in-hospital readmissions to the CICU within 48 hours of transfer.
• Using the Model for Improvement the team identified sources of failure in the CICU to the cardiac step-down transfer which contributed to <48-hour CICU readmission and the developed and tested a new care process practice bundle.
