  • A description of the models of care developed by The Christie NHS Foundation Trust to transform existing palliative care services into what the trust called enhanced supportive care (ESC). The care includes a multi-disciplinary ESC team which patients could access at any stage of their diagnosis or treatment and extended into multiple stand-alone ambulatory care clinics with rapid access for patients. 
  • Over the three-year trial, it was estimated almost 600 admissions were avoided by the ESC team. In the first six months ambulatory care clinics were shown to have avoided 141 patients admissions and 59 were discharged early.
